뒤통수 refers to the back of someone’s head and 치다 means ‘to hit’. The literal translation for this idiom would therefore be to hit the back of someone’s head.

뒤통수치다 is when you do something harmful to someone who trust you. You’d say or do one thing in front of that person, but behind their back and without their knowledge, you’d do the opposite. I guess you can say the phrase 뒤통수치다 carries the same meaning as the English idiom “stab someone in the back” where you betray or deceive someone when they least expect it.
